Job Description:

REQUISITE & PREFERRED SKILLS:

Experience in a healthcare setting; knowledge of community agencies and resources

ROLES AND RESPONSIBILITIES:

• Screens all patients referred to social work and assess psychosocial issues impacting health and develops treatment plans in conjunction with the patient and family to determine potential health/mental health care needs.
• Interviews and assesses all relevant patients and their situations which could also include meeting with families and/or significant others.
• Conducts a thorough biopsychosocial to evaluate the patient’s baseline physical, emotional and psychiatric needs and the impact of illness and hospitalization on his/her life.
• Assess the patient’s needs/eligibility for various economic, social or other concrete services.
• Attends interdisciplinary team rounds and maintains appropriate records as indicated.
• Participates in staff meetings.
• Complete all activities pertaining to the Healthy Steps / Collaborative Care Programs.
• Assists with linkages to community resources in conjunction with social work and medical providers.

Qualifications:

1. Master’s degree in social work      

2. LMSW required.
