The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. Location : Miami, FL 33165 & 33186. o Shift: 32-40 hrs. including Alternate weekends (8:30am – 6:30pm) Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning
The Telehealth Nurse Practitioner delivers patient care services through a remote technology platform. You will work in collaboration with a dedicated team of professionals as you independently provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients ranging in age 18 months and above.
o Location (mandatory): Aiken, SC 29803 Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning
Location : Roswell, Marietta, Douglasville in GA location. Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning
Location : Waynesville, NC 28786 and Hendersonville, NC 28792 Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning
Location : Fuquay Varina, North Carolina. Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
Location : Mooresville, NC Shift: This is a full-time contingent position, 32-40 hours per week including every other weekend, and may require floating to other nearby locations. Dress Code: Nursing Attire Nurses whose experience is primarily in long-term care settings such as nursing homes, home health, LTACs, and skilled nursing facilities may encounter challenges adapting to the clinic environment. Common areas of difficulty include managing multiple simultaneous tasks, limited exposure to standard immunization protocols, and unfamiliarity with navigating the EPIC electronic health record system.
