Contract - 03+ Months (Contract)
Candidate will travel approximately 75% of the time within the region seeing Members at home, in assisted living facilities and nursing homes. • We are seeking self-motivated, energetic, detail-oriented, highly organized, tech-savvy Case Management Coordinator to join our Case Management team. Our organization promotes autonomy through a Monday-Friday working schedule 8am-5pm and flexibility as you coordinate the care of your members. • Case Management Coordinator is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. • Case Management Coordinator will effectively manage a caseload that includes supportive and complex members. • Develop a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. • Case Management Coordinators will determine appropriate services and supports due to member’s health needs; including but not limited to: Prior Authorizations, Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and support. • Coordination with PCP and skilled providers, Condition management information, Medication review, Community resources and supports.
Location : Mooresville, IN 46158 Shift: Work 3-4 days per week, Hours M-F 8a-7p, Saturdays 9-5:30p, Sunday 9-4:30p; Training 40hrs x3 weeks Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
Location : Cincinnati, Dayton & West Chester in OHIO State Shift: Work 3-4 days per week, Hours M-F 8a-7p, Saturdays 9-5:30p, Sunday 9-4:30p; Training 40hrs x3 weeks Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
Location : Round Rock, TX 78664 Shift: This is a full-time contingent position, 32-40 hours per week including every other weekend, and may require floating to other nearby locations. As an LPN/ LVN, need experience in Primary Care, Urgent Care, or Emergency Room. Previous experience with EPIC Electronic Medical Record
Location : Austin, TX 78745 Shift: 65 hours per 2 week period or 32.50 hours per week Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
Location : Stafford, VA 22556 Shift: This is a full time contingent position, 32-40 hours per week including every other weekend, and may require floating to other nearby locations. Provide emergency care in collaboration with the provider on site in emergent situations. Perform patient registration, intake, and checkout. Assist with wait room management and other responsibilities such as patient reminder and other outreach calls, appointment scheduling, and chaperone for sensitive exams to enhance the patient experience.
