The CHW will act as a caring neighbor to help members address the social, medical and community problems that lead to poor health (SDoH/HRSN). As a priority, interactions will promote, maintain, and improve the health of members and their families. This includes to provide social support and advocate for individuals and community health needs. We work on a timeline to get these documents accurate, verified, and mailed, so time and precision are of the essence.
