Location : REMOTE with PA state RN License. Shift: Monday-Friday 8am-5pm EST Our Care Managers are frontline advocates for members who cannot advocate for themselves. They are responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness.

Please Note this is a 1 to 2 day a week position Under the general direction of the Regional Health Leader and within the general framework of the Health Services Operating Discipline and Clinical Guidelines, provide clinical care services, case management, health assessment program management and services, and general preventive health services for DuPont employees and specified contractor employees.
Location - Wilmington, DE 19801. Experienced and compassionate healthcare professionals skilled in conducting face-to-face and telephonic assessments for members with chronic and complex conditions. Proven ability to coordinate care across healthcare settings, develop individualized care plans, and connect members with community resources. Adept at supporting high-risk populations through effective care management strategies, documentation, and compliance with state and federal healthcare guidelines. This job works directly with providers in a variety of health care settings to appropriately identify members with chronic conditions and/or gaps in care that can be positively impacted in relation to quality and care costs. The incumbent could work in a physician’s office, visit physician practices on a routine basis, work within a hospital setting and/or visit the member’s home. This job directly helps members with the highest risk scores to coordinate care and navigate the healthcare system by recommending and/or implementing interventions related to the improvement of medical care and costs. Location : Mooresville, IN 46158 Shift: Work 3-4 days per week, Hours M-F 8a-7p, Saturdays 9-5:30p, Sunday 9-4:30p; Training 40hrs x3 weeks Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
Location : Cincinnati, Dayton & West Chester in OHIO State Shift: Work 3-4 days per week, Hours M-F 8a-7p, Saturdays 9-5:30p, Sunday 9-4:30p; Training 40hrs x3 weeks Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. Location : Round Rock, TX 78664 Shift: This is a full-time contingent position, 32-40 hours per week including every other weekend, and may require floating to other nearby locations. Location : Austin, TX 78745 Shift: 65 hours per 2 week period or 32.50 hours per week Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
Location : Stafford, VA 22556 Shift: This is a full time contingent position, 32-40 hours per week including every other weekend, and may require floating to other nearby locations. Provide emergency care in collaboration with the provider on site in emergent situations. Perform patient registration, intake, and checkout. Assist with wait room management and other responsibilities such as patient reminder and other outreach calls, appointment scheduling, and chaperone for sensitive exams to enhance the patient experience.
