Minimal one year of Dietician/ Nutritionist experience required. Under general direction of Director of Clinical Nutrition Services. To provide and evaluate nutritional care services to patients on assigned units. Adheres to hospital and departmental policies and procedures. Participates in all activities as a member of the health care team. Under adminustrative direction, participates in and/or organizes, plans and/or directs the operation of the dietary service.

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license
