Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidence-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for the entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will be reported to the practice manager.

Requires an RN, LCSW, or LCPC with an unrestricted active license. Experience with case management and IL waiver services is preferred.
The Med D Finance Coordinator role requires an ambitious, motivated, and eager individual looking to work for an industry-leading company. The position is in our PBM Finance Operations Member Experience department and will be part of the Medicare Coordination of Benefits (COB)/Medicare Secondary Payer (MSP) team, a group responsible for ensuring the appropriate adjudication of claims across multiple payers. As a Med D Coordinator, you will be responsible for reviewing and interpreting claims data on all adjudication platforms. You will take part in a variety of tasks and projects across the COB/MSP team. To be successful in this role, you will need to communicate effectively in individual and group situations, as well as the ability to plan assignments, develop action plans, and complete tasks in a timely manner.

This role is working remotely. This position will require travel to meet in person with members around the Service Delivery Area 75% of the time*** Utilizes critical thinking and judgment to collaborate and inform the service coordination process, in order to facilitate appropriate health care outcomes for members by providing service coordination, support and education for members using service coordination tools and resources.
