The Contractual Eligibility Consultant is responsible to research and validate enrollment information, processing enrollment transactions, make outbound calls to enrollees and members to obtain required enrollment information, handle member inquiries and participate in enrollment projects. The Contractual Eligibility Consultant will support training initiatives and work to resolve member inquiries or concerns

Location: CAPE CORAL FL 33904 & ESTERO FL 33928 (FLOAT). This is a full-time contingent position, starting at 30 hours per week up to a possible 40 hours, and may require floating to other nearby locations and working alternating weekends.

The care manager establishes a cost effective and member centric care plan in collaboration with the member, authorized care givers, and providers.

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.

Small Description: The Nurse Practitioner delivers defined patient care services in a retail clinic environment. You will work in collaboration with a dedicated team of professionals as you independently provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients ranging in age 18 months and above. Care is documented via computerized electronic health records. This position reports directly to the Senior Practice Manager. Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidence-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for the entire panel of patients. This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role will be reported to the practice manager.
