The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN, LCSW, or LCPC with an unrestricted active license. Experience with case management and IL waiver services is preferred.
Location (mandatory): Greensboro, NC 27410 Nurse Practitioner delivers defined patient care services in a retail clinic environment. You will work in collaboration with a dedicated team of professionals as you independently provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients ranging in age 18 months and above. Care is documented via computerized electronic health record
Location (mandatory): Newark, DE 19713 Facilitate the authorization process by obtaining timely authorizations; sends Admit Forms to Admitting Department to confirm insurance; schedule patients for imaging procedures including submitting referral information; prepares documents for Multidisciplinary Clinic (MDC) and enters in the EMR.
Location (mandatory): Louisville, KY Area The Care Management Associate supports comprehensive coordination of medical services including Care Team intake, screening and supporting the implementation of care plans to promote effective utilization of healthcare services. Promotes/supports quality effectiveness of Healthcare Services. This is a teleworking position, and this position is assigned to Kentucky. Qualified candidates must reside in Kentucky. 5% - 10% travel is required to the Louisville office for meetings, training, and administrative duties.
