The Nurse Practitioner delivers defined patient care services in a retail clinic environment. You will work in collaboration with a dedicated team of professionals as you independently provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients ranging in age 18 months and above. Care is documented via computerized electronic health records. This position reports directly to the Senior Practice Manager. Our clinic’s practice locations are located inside select Pharmacy and Target Stores. We are reinventing pharmacy with innovation, collaboration, caring, integrity, and accountability. Location (mandatory): Newark, DE 19713 Facilitate the authorization process by obtaining timely authorizations; sends Admit Forms to Admitting Department to confirm insurance; schedule patients for imaging procedures including submitting referral information; prepares documents for Multidisciplinary Clinic (MDC) and enters in the EMR.
Location (mandatory): Louisville, KY Area The Care Management Associate supports comprehensive coordination of medical services including Care Team intake, screening and supporting the implementation of care plans to promote effective utilization of healthcare services. Promotes/supports quality effectiveness of Healthcare Services. This is a teleworking position, and this position is assigned to Kentucky. Qualified candidates must reside in Kentucky. 5% - 10% travel is required to the Louisville office for meetings, training, and administrative duties.
Work Schedule: Monday through Friday, 9:30am – 6pm As a Pharmacy Technician in a growing segment of healthcare, specialty pharmacy, you will work with specialized and complex therapies that are delivered to patients.

Work Schedule: M-F 8-5 CST IL resident who lives in one of the counties listed below and is interested in doing field case management with the elderly and individuals with disabilities who are approved for in-home or nursing facility care. Kankakee or Northside Chicago counties in Illinois
